Evaluating your own work term experience and having employers evaluate your performance are required components of accredited co-op programs. |
| Evaluation by Students of Work Term | After each work term, please complete the evaluation by students of work term form and give it to your coordinator before or during your back-to-campus appointment. |
| Performance Evaluation Form - Word | Near the end of your work term, please have your supervisor complete the performance evaluation form and return it to the Co-op Office. |
